This sequence change replaces isoleucine with valine at codon 1723 of the BRCA1 protein (p.Ile1723Val). The isoleucine residue is moderately conserved and there is a small physicochemical difference between isoleucine and valine. This variant is not present in population databases (ExAC no frequency). This variant has been reported in individuals affected with breast cancer in the Leiden Open-source Variation Database (PMID: 21520333). However, in one of these individuals a pathogenic allele was also identified in BRCA2, which suggests that this c.5167A>G variant was not the primary cause of disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies. In summary, this variant is a rare missense change that is not predicted to affect protein function. While it is absent from the population and reported in affected individuals, it has also been shown to co-occur with a pathogenic variant in BRCA2 in an affected individual. The available evidence is currently insufficient to determine its role in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
