BRCA1 (HGNC:1100): (BRCA1 DNA repair associated) This gene encodes a 190 kD nuclear phosphoprotein that plays a role in maintaining genomic stability, and it also acts as a tumor suppressor. The BRCA1 gene contains 22 exons spanning about 110 kb of DNA. The encoded protein combines with other tumor suppressors, DNA damage sensors, and signal transducers to form a large multi-subunit protein complex known as the BRCA1-associated genome surveillance complex (BASC). This gene product associates with RNA polymerase II, and through the C-terminal domain, also interacts with histone deacetylase complexes. This protein thus plays a role in transcription, DNA repair of double-stranded breaks, and recombination. Mutations in this gene are responsible for approximately 40% of inherited breast cancers and more than 80% of inherited breast and ovarian cancers. Alternative splicing plays a role in modulating the subcellular localization and physiological function of this gene. Many alternatively spliced transcript variants, some of which are disease-associated mutations, have been described for this gene, but the full-length natures of only some of these variants has been described. Of note, loss-of-function variants in select exons of BRCA1 have been shown to retain some of their tumor suppressor function (PMID: 27008870, 32046981). This variant is absent in gnomAD v2.1.1 (https://gnomad.broadinstitute.org/) and in the FLOSSIES database which contains genetic variants from women older than 70 years of age who have never had cancer (https://whi.color.com/). In summary, the evidence currently available is insufficient to determine the clinical significance of this variant. It has therefore been classified as of uncertain significance. -

This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site, however a known cryptic splice acceptor site 3 nucleotides downstream of the native acceptor site is predicted to be maintained. RNA studies indicate the presence of a transcript using this alternate acceptor site, predicted to result in the in-frame loss of a single amino acid (p.Gln1396del), though this transcript is naturally occurring and is also present in controls (Ambry internal data and Colombo M et al. Hum Mol Genet, 2014 Jul;23:3666-80), therefore the clinical impact is uncertain. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Therefore, it has been classified as a Variant of Uncertain Significance. Donor and acceptor splice site variants typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in BRCA1 are known to be pathogenic (PMID: 20104584). Experimental studies have shown that variants at this splice site disrupt the consensus splice site and strengthen a cryptic acceptor site in exon 12 (also known as exon 13 in the literature), located 3 nucleotides downstream of the natural splice site. This results in an in-frame deletion of 1 amino acid (p.Gln1396del), but otherwise preserves the integrity of the reading frame (PMID: 24569164). These studies suggest that the clinical significance of this splice variant may be uncertain. This variant has not been reported in the literature in individuals with BRCA1-related conditions. ClinVar contains an entry for this variant (Variation ID: 240799). This variant is not present in population databases (ExAC no frequency). This sequence change affects an acceptor splice site in intron 11 of the BRCA1 gene. It is expected to disrupt RNA splicing and likely results in an absent or disrupted protein product. -
