Genes affected
BRCA1 (HGNC:1100): (BRCA1 DNA repair associated) This gene encodes a 190 kD nuclear phosphoprotein that plays a role in maintaining genomic stability, and it also acts as a tumor suppressor. The BRCA1 gene contains 22 exons spanning about 110 kb of DNA. The encoded protein combines with other tumor suppressors, DNA damage sensors, and signal transducers to form a large multi-subunit protein complex known as the BRCA1-associated genome surveillance complex (BASC). This gene product associates with RNA polymerase II, and through the C-terminal domain, also interacts with histone deacetylase complexes. This protein thus plays a role in transcription, DNA repair of double-stranded breaks, and recombination. Mutations in this gene are responsible for approximately 40% of inherited breast cancers and more than 80% of inherited breast and ovarian cancers. Alternative splicing plays a role in modulating the subcellular localization and physiological function of this gene. Many alternatively spliced transcript variants, some of which are disease-associated mutations, have been described for this gene, but the full-length natures of only some of these variants has been described. A related pseudogene, which is also located on chromosome 17, has been identified. [provided by RefSeq, May 2020]

Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsSep 11, 2023The p.T1349R variant (also known as c.4046C>G), located in coding exon 9 of the BRCA1 gene, results from a C to G substitution at nucleotide position 4046. The threonine at codon 1349 is replaced by arginine, an amino acid with similar properties. In a study of 15 paired tumor and normal tissue samples from women with breast cancer, this alteration was detected in one individual, and loss of heterozygosity was not observed in her breast tumor sample (Beristain E et al. Fam. Cancer, 2010 Sep;9:289-90). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Uncertain significance, criteria provided, single submitterclinical testingColor Diagnostics, LLC DBA Color HealthAug 30, 2021This missense variant replaces threonine with arginine at codon 1349 of the BRCA1 protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with breast and/or ovarian cancer (PMID: 17262179) and also has been reported with tumor pathology (1 tumor sample) and family history (1 pedigree) likelihood ratios for pathogenicity of 0.34 and 0.4825, respectively (PMID: 31131967). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Uncertain significance, criteria provided, single submitterclinical testingGenetic Services Laboratory, University of ChicagoApr 01, 2021DNA sequence analysis of the BRCA1 gene demonstrated a sequence change, c.4046C>G, in exon 10 that results in an amino acid change, p.Thr1349Arg. This sequence change has been previously described in one individual with breast/ovarian cancer (PMID: 17262179). This sequence change is absent in the gnomAD population database. The p.Thr1349Arg change affects a moderately conserved amino acid residue located in a domain of the BRCA1 protein that is known to be functional. In-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL) provide contradictory results for the p.Thr1349Arg substitution. Due to these contrasting evidences and the lack of functional studies, the clinical significance of the p.Thr1349Arg change remains unknown at this time. -
