The p.D1343V variant (also known as c.4028A>T), located in coding exon 9 of the BRCA1 gene, results from an A to T substitution at nucleotide position 4028. The aspartic acid at codon 1343 is replaced by valine, an amino acid with highly dissimilar properties. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

Variant summary: BRCA1 c.4028A>T (p.Asp1343Val) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 4e-06 in 251164 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.4028A>T has been observed in at-least one individual younger than 40 years who was affected with breast cancer, without providing supportive evidence for causality (Abdel-Razeq_2021), as well as an individual with bile duct cancer (Ando_2024). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 34290354, 38781545). ClinVar contains an entry for this variant (Variation ID: 409318).
