The BRCA1 c.2936G>A (p.Arg979His) variant has been identified in the published literature in reportedly healthy individuals (PMID: 31214711 (2020), 30287823 (2018)), and described to be located in a region of the BRCA1 gene that is tolerant to missense sequence changes (PMID: 31911673 (2020)). To the best of our knowledge, this variant has not been reported in individuals with BRCA1-related disorders. The frequency of this variant in the general population, 0.000014 (4/282498 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

Variant summary: BRCA1 c.2936G>A (p.Arg979His) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.4e-05 in 282498 control chromosomes. The variant has also been reported in Japanese controls (Momozawa_2018). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, c.2936G>A has not been reported in the literature in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome nor has experimental evidence demonstrating an impact on protein function has been reported. Four clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. Two submitters have classified the variant as VUS while two have classified as likely benign. Based on the evidence outlined above, the variant was classified as uncertain significance. -

This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 979 of the BRCA1 protein (p.Arg979His). This variant is present in population databases (rs80356985, gnomAD 0.008%). This variant has not been reported in the literature in individuals affected with BRCA1-related conditions. ClinVar contains an entry for this variant (Variation ID: 54728). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA1 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
