BRCA1 (HGNC:1100): (BRCA1 DNA repair associated) This gene encodes a 190 kD nuclear phosphoprotein that plays a role in maintaining genomic stability, and it also acts as a tumor suppressor. The BRCA1 gene contains 22 exons spanning about 110 kb of DNA. The encoded protein combines with other tumor suppressors, DNA damage sensors, and signal transducers to form a large multi-subunit protein complex known as the BRCA1-associated genome surveillance complex (BASC). This gene product associates with RNA polymerase II, and through the C-terminal domain, also interacts with histone deacetylase complexes. This protein thus plays a role in transcription, DNA repair of double-stranded breaks, and recombination. Mutations in this gene are responsible for approximately 40% of inherited breast cancers and more than 80% of inherited breast and ovarian cancers. Alternative splicing plays a role in modulating the subcellular localization and physiological function of this gene. Many alternatively spliced transcript variants, some of which are disease-associated mutations, have been described for this gene, but the full-length natures of only some of these variants has been described. The BRCA1 p.F461L variant was reported in the literature in an individual with breast cancer and two individuals with prostate cancer (Katagiri_1998_PMID: 9609997, Pugh_2009_PMID: 19638463). The variant was identified in dbSNP (ID: rs62625300) and ClinVar (classified as uncertain significance by Invitae, Ambry Genetics, GeneDx and six other submitters). The variant was identified in control databases in 1 of 251080 chromosomes at a frequency of 0.000003983 (Genome Aggregation Database March 6, 2019, v2.1.1). The p.F461 residue is conserved in mammals and computational analyses (MUT Assesor, PolyPhen-2, SIFT, MutationTaster, Revel, FATHMM, MetaLR, DANN) suggest that the variant may impact the protein; however, this information is not predictive enough to assume pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (Splice AI exome) do not predict a deleterious effect on splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. The p.F461L variant (also known as c.1381T>C), located in coding exon 9 of the BRCA1 gene, results from a T to C substitution at nucleotide position 1381. The phenylalanine at codon 461 is replaced by leucine, an amino acid with highly similar properties. This alteration was previously identified in one kindred of Japanese descent with gastric and early onset breast cancer (Katagiri T et al. J. Hum. Genet. 1998; 43(1):42-8). This alteration was also identified in a high-risk breast and/or ovarian cancer family (Lu W et al. Fam Cancer, 2012 Sep;11:381-5). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
