BRCA1 (HGNC:1100): (BRCA1 DNA repair associated) This gene encodes a 190 kD nuclear phosphoprotein that plays a role in maintaining genomic stability, and it also acts as a tumor suppressor. The BRCA1 gene contains 22 exons spanning about 110 kb of DNA. The encoded protein combines with other tumor suppressors, DNA damage sensors, and signal transducers to form a large multi-subunit protein complex known as the BRCA1-associated genome surveillance complex (BASC). This gene product associates with RNA polymerase II, and through the C-terminal domain, also interacts with histone deacetylase complexes. This protein thus plays a role in transcription, DNA repair of double-stranded breaks, and recombination. Mutations in this gene are responsible for approximately 40% of inherited breast cancers and more than 80% of inherited breast and ovarian cancers. Alternative splicing plays a role in modulating the subcellular localization and physiological function of this gene. Many alternatively spliced transcript variants, some of which are disease-associated mutations, have been described for this gene, but the full-length natures of only some of these variants has been described. The p.V419L variant (also known as c.1255G>C), located in coding exon 9 of the BRCA1 gene, results from a G to C substitution at nucleotide position 1255. The valine at codon 419 is replaced by leucine, an amino acid with highly similar properties. This alteration was identified in an individual with a personal and family history of breast cancer (Carney ME et al. Hawaii Med J 2010 Nov; 69(11):268-71). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

To our knowledge, functional studies have not been reported for this variant. This variant has been detected in a breast cancer case-control meta-analysis in 0/60466 cases and 2/53461 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID BRCA1_006522), and it has also been reported in an individual affected with breast or ovarian cancer (PMID: 28364669). A multifactorial analysis has reported a likelihood ratio for pathogenicity based on personal and family history of 0.23 from log(LR)=-0.6378 for two carriers (PMID: 31853058). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Variant summary: The BRCA1 c.1255G>C (p.Val419Leu) variant involves the alteration of a non-conserved nucleotide. 3/5 in silico tools predict a benign outcome; functional studies had not been published at the time of variant classification. This variant was absent in 121208 control chromosomes, but was reported in one breast cancer patient diagnosed at 72 years old without evidence of causality (i.e. co-segregation data). Additionally, one clinical lab classified the variant as a VUS. Taken together, the variant was classified as a variant of uncertain significance (VUS) until additional information becomes available. -

The p.Leu419Val variant has been reported in the literature in 1 of 546 proband chromosomes in an individual with breast cancer with late onset (Carney_2010).
