This variant disrupts the p.Arg258 amino acid residue in GFAP. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 11138011, 23925719, 28448978). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). This variant has been observed in individual(s) with Alexander disease (PMID: 28448978, 29443213). This variant is present in population databases (rs61726468, ExAC 0.02%). This sequence change replaces arginine with histidine at codon 258 of the GFAP protein (p.Arg258His). The arginine residue is highly conserved and there is a small physicochemical difference between arginine and histidine. -
