The acetylcholine receptor changes subunit composition shortly after birth when the epsilon subunit replaces the gamma subunit seen in embryonic receptors. This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 340 of the CHRNE protein (p.Pro340Arg). This variant is present in population databases (rs139922367, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with CHRNE-related conditions. ClinVar contains an entry for this variant (Variation ID: 970144). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt CHRNE protein function with a negative predictive value of 95%. This alteration results from a C to G substitution at nucleotide position 1019, causing the proline (P) at amino acid position 340 to be replaced by an arginine (R).
