Variant summary: RAD51C c.718A>T (p.Ile240Leu) results in a conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251402 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.718A>T has been reported in the literature as a VUS in an individual affected with colorectal cancer (Yurgelun_2017). This report does not provide unequivocal conclusions about association of the variant with Fanconi Anemia Complementation Group O. At least one publication reports experimental evidence evaluating an impact on protein function (Hu_2023). These results showed no damaging effect of this variant in an HDR experiment. The following publications has been ascertained in the context of this evaluation (PMID: 28135145, 37253112). ClinVar contains an entry for this variant (Variation ID: 216808). Based on the evidence outlined above, the variant was classified as uncertain significance. -
Hereditary cancer-predisposing syndrome Uncertain:2
This missense variant replaces isoleucine with leucine at codon 240 of the RAD51C protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with colorectal cancer (PMID: 28135145). In a large breast cancer case-control study, this variant was identified in 1/60465 cases and was absent in any controls (PMID: 33471991 - Leiden Open Variation Database DB-ID RAD51C_000178). This variant has been identified in 1/251402 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.I240L variant (also known as c.718A>T), located in coding exon 5 of the RAD51C gene, results from an A to T substitution at nucleotide position 718. The isoleucine at codon 240 is replaced by leucine, an amino acid with highly similar properties. This variant has been reported in an individual affected with colorectal cancer (Yurgelun MB et al. J Clin Oncol, 2017 Apr;35:1086-1095). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Published functional studies demonstrate homology-directed repair activity similar to wildtype (PMID: 37253112); In silico analysis supports that this missense variant does not alter protein structure/function; Not observed at significant frequency in large population cohorts (gnomAD); Observed in individual(s) with colorectal cancer (PMID: 28135145); This variant is associated with the following publications: (PMID: 14704354, 33471991, 37253112, 28135145) -
RAD51C-related disorder Uncertain:1
The RAD51C c.718A>T variant is predicted to result in the amino acid substitution p.Ile240Leu. This variant was reported as uncertain significance in an individual with colorectal cancer (Table A4, Yurgelun et al. 2017. PubMed ID: 28135145). This variant is reported in 0.00088% of alleles in individuals of European (Non-Finnish) descent in gnomAD (http://gnomad.broadinstitute.org/variant/17-56787232-A-T) and is interpreted as uncertain significance in the ClinVar database (https://www.ncbi.nlm.nih.gov/clinvar/variation/216808/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
Fanconi anemia complementation group O Uncertain:1
This sequence change replaces isoleucine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 240 of the RAD51C protein (p.Ile240Leu). This variant is present in population databases (no rsID available, gnomAD 0.0009%). This missense change has been observed in individual(s) with colorectal cancer (PMID: 28135145). ClinVar contains an entry for this variant (Variation ID: 216808). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt RAD51C protein function with a negative predictive value of 80%. Experimental studies have shown that this missense change does not substantially affect RAD51C function (PMID: 37253112). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
