The p.R777H variant (also known as c.2330G>A), located in coding exon 15 of the BRIP1 gene, results from a G to A substitution at nucleotide position 2330. The arginine at codon 777 is replaced by histidine, an amino acid with highly similar properties. This alteration has been observed in multiple individuals with a personal history of breast cancer and absent in controls (Easton DF et al. J Med Genet, 2016 05;53:298-309). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
This missense variant replaces arginine with histidine at codon 777 of the BRIP1 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been detected in a breast cancer case-control meta-analysis in 3/60466 cases and 0/53461 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID BRIP1_000423). This variant has been identified in 6/251306 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
PP3_Moderate c.2330G>A , located in exon 16 of the BRIP1 gene, is predicted to result in the substitution of Arginine by Histidine at codon 777, p.(Arg777His), This variant is found in 6/268166 alleles at a frequency of 0.0024% in the gnomAD v2.1.1 database, non-cancer dataset. The SpliceAI algorithm predicts no significant impact on splicing. The REVEL meta-predictor score for this variant (0.867) suggests a deleterious effect on protein function according to Pejaver 2022 thresholds (PMID: 36413997) (PP3_Moderate). To our knowledge, neither relevant clinical data nor well-stablished functional studies have been reported for this variant. It has been reported in ClinVar (4x as uncertain significance) and in the LOVD database (1x NA, 1x uncertain significancer). Based on currently available information, the variant c.2330G>A should be considered an uncertain significance variant. -

The BRIP1 c.2330G>A (p.Arg777His) variant has been reported In large case-control studies, this variant was detected in individuals with breast cancer and not in any reportedly healthy controls (PMIDs: 26921362 (2016) and 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/genes/BRIP1)). The frequency of this variant in the general population, 0.000087 (3/34580 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 777 of the BRIP1 protein (p.Arg777His). This variant is present in population databases (rs747568830, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with BRIP1-related conditions. ClinVar contains an entry for this variant (Variation ID: 230681). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt BRIP1 protein function with a positive predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
