The BRIP1 c.1187A>G (p.His396Arg) variant has been reported in the published literature in an individual with male breast cancer (PMID: 32885271 (2021)) as well as in reportedly healthy individuals (PMID: 26315354 (2015)). The frequency of this variant in the general population, 0.0000071 (2/282116 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Based on the available information, we are unable to determine the clinical significance of this variant.

The p.H396R variant (also known as c.1187A>G), located in coding exon 8 of the BRIP1 gene, results from an A to G substitution at nucleotide position 1187. The histidine at codon 396 is replaced by arginine, an amino acid with highly similar properties. This variant was detected in an individual diagnosed with breast cancer at age 73 who met eligibility criteria for hereditary breast and ovarian cancer syndrome (Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879). In one study, this variant was seen in 0/3236 cases with invasive epithelial ovarian cancer and in 2/3431 controls (Ramus SJ et al. J Natl Cancer Inst, 2015 Nov;107). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
