This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 251 of the BRIP1 protein (p.Arg251His). This variant is present in population databases (rs780834054, gnomAD 0.009%). This missense change has been observed in individual(s) with breast cancer, clinical features of Lynch syndrome, and/or thyroid cancer (PMID: 25980754, 35534704). ClinVar contains an entry for this variant (Variation ID: 232270). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on BRIP1 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The p.R251H variant (also known as c.752G>A), located in coding exon 6 of the BRIP1 gene, results from a G to A substitution at nucleotide position 752. The arginine at codon 251 is replaced by histidine, an amino acid with highly similar properties. In one study, this alteration was not identified in 706 ovarian cancer patients or 6,341 breast cancer patients, but was reported in 3 of 36,687 controls (Weber-Lassalle N et al. Breast Cancer Res., 2018 Jan;20:7). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

This variant has not, to our knowledge, been published in the literature as either a pathogenic variant or a benign polymorphism. BRIP1 Arg251His was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Since Arginine and Histidine share similar properties, this is considered a conservative amino acid substitution. BRIP1 Arg251His alters a position that is highly conserved across species and is located within the helicase ATP-binding domain (UniProt). In silico analyses predict that this variant is probably damaging to protein structure and function. Based on currently available information, it is unclear whether BRIP1 Arg251His is a pathogenic or a benign variant.
