BP4_Moderate c.679C>G, located in exon 7 of the BRIP1, is predicted to result in the substitution of glutamine with glutamic acid at codon 227, p.(Gln227Glu). This variant is found in 8/102350, with a filter allele frequency of 0.003% at 99% confidence in the gnomAD v2.1.1 database (European non-Finnish non-cancer data set). The SpliceAI algorithm predicts no significant impact on splicing and the REVEL meta-predictor score for this variant (0.046) suggests that it does not affect the protein function according to Pejaver 2022 thresholds (PMID: 36413997) (BP4_Moderate). To our knowledge, neither relevant clinical data nor functional studies have been reported for this variant. It has been reported in the ClinVar database (1x likely benign, 13x uncertain significance) and in the LOVD database (1x likely benign, 2x not classified). Based on the currently available information, c.679C>G is classified as an uncertain significance variant according to ACMG guidelines.

This missense variant replaces glutamine with glutamic acid at codon 227 of the BRIP1 protein. Computational prediction suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in several individuals affected with breast cancer, as well as in an unaffected control individual (PMID: 26921362, 31822495). In a large breast cancer case-control meta-analysis, this variant has been observed in 5/60466 cases and 5/53461 unaffected controls (OR=0.884, 95%CI 0.256 to 3.054; PMID: 33471991; Leiden Open Variation Database DB-ID BRIP1_000027). This variant has been identified in 9/250948 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

The BRIP1 c.679C>G (p.Gln227Glu) variant has been reported in the published literature in individuals affected with breast cancer (PMID: 32658311 (2021), 33471991 (2021), see also LOVD (https://databases.lovd.nl/shared/variants/BRIP1), 31822495 (2020)). This variant is also found in reportedly healthy individuals (PMID: 33471991 (2021), see also LOVD (https://databases.lovd.nl/shared/variants/BRIP1), 17033622 (2006)). The frequency of this variant in the general population, 0.000079 (9/113360 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant.
