The L796V variant has been reported previously as a de novo variant in an individual with paramyotonia congenita; however, additional clinical information was not provided and functional characterization of the variant was not completed (Kim, J.B. 2010). It was not observed in approximately 6,200 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The L796V variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. However, this substitution alters a highly conserved position predicted to be in the transmembrane segment S6 of the second homologous domain of the SCN4A protein. Additionally, in silico analysis predicts this variant is probably damaging to the protein structure/function. Furthermore, a missense variant in a nearby residue (A799S) has been reported in the Human Gene Mutation Database in association with an SCN4A-related disorder (Stenson et al., 2014). Therefore, this variant is likely pathogenic; however, the possibility that it is benign cannot be excluded. GeneDx interprets L796V as a likely pathogenic variant. -

ClinVar contains an entry for this variant (Variation ID: 383923). This missense change has been observed in individual(s) with SCN4A-related conditions (PMID: 31609695, 31732390, 32117035). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces leucine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 796 of the SCN4A protein (p.Leu796Val). -
