Variant summary

Our verdict is Pathogenic. The variant received 14 ACMG points: 14P and 0B. PS3PM1PM2PM5PP3_Strong

The NM_000546.6(TP53):c.829T>C(p.Cys277Arg) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000137 in 1,461,858 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★★). ClinVar reports functional evidence for this variant: "SCV000571115: multiple functional studies consistently associated TP53 Cys277Arg with decreased DNA binding specificity and reduced overall transactivation activity (Thukral 1995, Saller 1999, Kaesar 2002, Resnick 2003). Additionally, this variant is reported as having non-functional transactivation in the International Agency for Research on Cancer TP53 database based on functional assays by Kato et al. (2003)." and additional evidence is available in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. C277F) has been classified as Likely pathogenic. The gene TP53 is included in the ClinGen Criteria Specification Registry.

Genes affected
TP53 (HGNC:11998): (tumor protein p53) This gene encodes a tumor suppressor protein containing transcriptional activation, DNA binding, and oligomerization domains. The encoded protein responds to diverse cellular stresses to regulate expression of target genes, thereby inducing cell cycle arrest, apoptosis, senescence, DNA repair, or changes in metabolism. Mutations in this gene are associated with a variety of human cancers, including hereditary cancers such as Li-Fraumeni syndrome. Alternative splicing of this gene and the use of alternate promoters result in multiple transcript variants and isoforms. Additional isoforms have also been shown to result from the use of alternate translation initiation codons from identical transcript variants (PMIDs: 12032546, 20937277). ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 14 ACMG points.

PS3
PS3 evidence extracted from ClinVar submissions: SCV000571115: multiple functional studies consistently associated TP53 Cys277Arg with decreased DNA binding specificity and reduced overall transactivation activity (Thukral 1995, Saller 1999, Kaesar 2002, Resnick 2003). Additionally, this variant is reported as having non-functional transactivation in the International Agency for Research on Cancer TP53 database based on functional assays by Kato et al. (2003).; SCV002679215: Functional studies have demonstrated a complete loss of transactivation capacity compared to wild type p53 protein (Petitjean A et al. IARC TP53 database [version R17, November 2013]. Hum Mutat. 2007 Jun;28(6):622-9), and altered DNA binding specificity (Saller E et al. EMBO J., 1999 Aug;18:4424-37; Kaeser MD et al. Proc. Natl. Acad. Sci. U.S.A., 2002 Jan;99:95-100).
PM1
In a hotspot region, there are 66 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 6 benign, 29 uncertain in NM_000546.6
