Genes affected
PIEZO2 (HGNC:26270): (piezo type mechanosensitive ion channel component 2) The protein encoded by this gene contains more than thirty transmembrane domains and likely functions as part of mechanically-activated (MA) cation channels. These channels serve to connect mechanical forces to biological signals. The encoded protein quickly adapts MA currents in somatosensory neurons. Defects in this gene are a cause of type 5 distal arthrogryposis. Several alternatively spliced transcript variants of this gene have been described, but their full-length nature is not known. ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 18 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, LoF is a know mechanism of disease, Cryptic splice site detected, with MaxEntScore 6.7, offset of 1, new splice context is: aatgtctcattctcctcaAGgca. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in frameshift change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 18-10714931-C-T is Pathogenic according to our data. Variant chr18-10714931-C-T is described in ClinVar as [Pathogenic]. Clinvar id is 489219.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Pathogenic, criteria provided, single submitterclinical testingGeneDxDec 07, 2017The c.5083-1G>A variant in the PIEZO2 gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This splice site variant destroys the canonical splice acceptor site in intron 35. It is predicted to cause abnormal gene splicing, either leading to an abnormal message that is subject to nonsense-mediated mRNA decay, or to an abnormal protein product if the message is used for protein translation. The c.5083-1G>A variant is not observed in large population cohorts (Lek et al., 2016). We interpret c.5083-1G>A as a pathogenic variant. -

Pathogenic, criteria provided, single submitterclinical testingUndiagnosed Diseases Network, NIHDec 11, 2017This splice site mutation is categorized as deleterious according to ACMG guidelines (PMID: 18414213) and was found in trans with another variant (c.1528-1G>A) in a 6-year-old female with ataxia, hypotonia, mild dysmorphic features, delayed motor development, scoliosis. It is predicted to cause abnormal gene splicing, either leading to an abnormal message that is subject to nonsense-mediated mRNA decay, or to an abnormal protein product if the message is used for protein translation. -
