Variant summary

Our verdict is Pathogenic. The variant received 21 ACMG points: 21P and 0B. PS3PM1PM2PP2PP3_StrongPP5_Very_Strong

The NM_000527.5(LDLR):c.917C>T(p.Ser306Leu) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000206 in 1,456,922 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV002774350: Functional studies of patient cells with this variant showed a substantial decrease in LDLR activity (PMID:1301956 (1992))." and additional evidence is available in ClinVar. The gene LDLR is included in the ClinGen Criteria Specification Registry.

Genes affected
LDLR (HGNC:6547): (low density lipoprotein receptor) The low density lipoprotein receptor (LDLR) gene family consists of cell surface proteins involved in receptor-mediated endocytosis of specific ligands. The encoded protein is normally bound at the cell membrane, where it binds low density lipoprotein/cholesterol and is taken into the cell. Lysosomes release the cholesterol, which is made available for repression of microsomal enzyme 3-hydroxy-3-methylglutaryl coenzyme A (HMG CoA) reductase, the rate-limiting step in cholesterol synthesis. At the same time, a reciprocal stimulation of cholesterol ester synthesis takes place. Mutations in this gene cause the autosomal dominant disorder, familial hypercholesterolemia. Alternate splicing results in multiple transcript variants.[provided by RefSeq, May 2022]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 21 ACMG points.

PS3
PS3 evidence extracted from ClinVar submissions: SCV002774350: Functional studies of patient cells with this variant showed a substantial decrease in LDLR activity (PMID: 1301956 (1992)).; SCV005870215: Published functional studies suggest a damaging effect, demonstrating a substantial decrease in LDLR activity (PMID: 1301956);; SCV001344023: An experimental functional study has shown that this variant causes significant decrease in LDLR activity (PMID: 1301956).; SCV005888168: The most pronounced variant effect results in LDL receptor activity of 2-5% (Hobbs_1992).
PM1
In a hotspot region, there are 26 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 1 benign, 26 uncertain in NM_000527.5
PM2
Very rare variant in population databases, with high coverage;
PP2
Missense variant in the gene, where a lot of missense mutations are associated with disease in ClinVar. The gene has 678 curated pathogenic missense variants (we use a threshold of 10). The gene has 92 curated benign missense variants. Gene score misZ: 0.12407 (below the threshold of 3.09). Trascript score misZ: 0.59338 (below the threshold of 3.09). GenCC associations: The gene is linked to hypercholesterolemia, familial, 1, homozygous familial hypercholesterolemia.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.982
PP5
Variant 19-11107491-C-T is Pathogenic according to our data. Variant chr19-11107491-C-T is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 226337.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
