This missense variant replaces cysteine with glycine at codon 364 in the LDLR EGF-like repeat B of the LDLR protein. This variant is also known as p.Cys343Gly in the mature protein. This variant alters a conserved cysteine residue that is critical for proper protein folding and function (PMID: 2088165, 6091915, 15952897). Computational prediction suggests that this variant may have a deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with LDLR-related disorders in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Multiple different variants affecting the same codon: p.Cys364Arg, p.Cys364Tyr, p.Cys364Phe, and p.Cys364Ser; are considered to be disease-causing (ClinVar variation ID: 251657, 369852, 251659, 251658), suggesting that cysteine at this position is important for LDLR protein function. Based on the available evidence, this variant is classified as Likely Pathogenic. -

The p.C364G variant (also known as c.1090T>G), located in coding exon 8 of the LDLR gene, results from a T to G substitution at nucleotide position 1090. The cysteine at codon 364 is replaced by glycine, an amino acid with highly dissimilar properties. Pathogenic LDLR mutations that result in the substitution or generation of cysteine residues within the cysteine-rich LDLR class A repeats and EGF-like domains are common in familial hypercholesterolemia (FH) (Villéger L. Hum Mutat. 2002;20(2):81-7). Internal structural analysis indicates this variant eliminates a disulfide bond critical for the structural integrity of the EGF-like 2 domain (Ambry internal data). Another alteration at the same codon, p.C364R (c.1090T>C), has also been reported in association with FH (Hobbs HH et al. Hum Mutat. 1992;1(6):445-66; Bertolini S et al. Arterioscler Thromb Vasc Biol. 2000;20(9):E41-52). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
