LDLR (HGNC:6547): (low density lipoprotein receptor) The low density lipoprotein receptor (LDLR) gene family consists of cell surface proteins involved in receptor-mediated endocytosis of specific ligands. The encoded protein is normally bound at the cell membrane, where it binds low density lipoprotein/cholesterol and is taken into the cell. Lysosomes release the cholesterol, which is made available for repression of microsomal enzyme 3-hydroxy-3-methylglutaryl coenzyme A (HMG CoA) reductase, the rate-limiting step in cholesterol synthesis. At the same time, a reciprocal stimulation of cholesterol ester synthesis takes place. Mutations in this gene cause the autosomal dominant disorder, familial hypercholesterolemia. The methionine at codon 531 is replaced by arginine, an amino acid with similar properties. This variant has been detected in an individual with heterozygous familial hypercholesterolemia (HeFH); however, clinical details were limited (Wang J et al. Arterioscler Thromb Vasc Biol, 2016 12;36:2439-2445). Another alteration at the same codon, p.M531K (c.1592T>A), has been described in two hypercholesterolemia probands and showed some evidence of functional impact (Charng MJ et al. Eur J Clin Invest, 2006 Dec;36:866-74; Hsiung YC et al. Atherosclerosis, 2018 10;277:440-447). Based on internal structural analysis, the variant is moderately destabilizing to the local structure. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

ClinVar contains an entry for this variant (Variation ID: 226362). This missense change has been observed in individual(s) with autosomal dominant familial hypercholesterolemia (PMID: 27765764, 33303402). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces methionine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 531 of the LDLR protein (p.Met531Arg). -
