This variant is a published mutation that has been seen in a patient with microcephaly with early-onset, intractable seizures and developmental delay (MCSZ) (Shen et al., 2010). The E326K mutation results in a PNKP protein that is less stable than the wild-type enzyme at physiological temperature and results in greatly reduced cellular DNA 3'-phosphatase activity (Reynolds et al., 2012). The variant is found in PNKP panel(s). -

This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 326 of the PNKP protein (p.Glu326Lys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of PNKP-related conditions (PMID: 20118933, 32056211). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 4846). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on PNKP protein function. Studies have shown that this missense change alters PNKP gene expression (PMID: 20118933). For these reasons, this variant has been classified as Pathogenic. -
