Variant summary
Our verdict is Uncertain significance. The variant received 2 ACMG points: 2P and 0B. PM2
The NM_002691.4(POLD1):c.2606C>A(p.Ser869*) variant causes a stop gained change. The variant allele was found at a frequency of 0.000000685 in 1,460,808 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. S869S) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Colorectal cancer, susceptibility to, 10 Uncertain:1
Missense variants that disrupt the 3'-5' exonuclease (proof-reading) activity of the POLD1 protein, are associated with an increased risk for colonic adenomatous polyps and colon cancer (PMID: 23263490, 23447401). However loss-of-function variants, which result in an absent or severely disrupted POLD1 protein, and missense variants outside the exonuclease domain, are unlikely to be associated with polyposis or colon cancer. Without further clinical and genetic evidence, this variant has been classified as a Variant of Uncertain Significance. This variant has not been reported in the literature in individuals with POLD1-related conditions. ClinVar contains an entry for this variant (Variation ID: 821538). This variant is not present in population databases (ExAC no frequency). This sequence change creates a premature translational stop signal (p.Ser869*) in the POLD1 gene. It is expected to result in an absent or disrupted protein product. -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.S869* variant (also known as c.2606C>A), located in coding exon 20 of the POLD1 gene, results from a C to A substitution at nucleotide position 2606. This changes the amino acid from a serine to a stop codon within coding exon 20. This alteration is expected to result in protein truncation or nonsense-mediated mRNA decay. However, loss of function of POLD1 has not been established as a mechanism of disease. Based on the available evidence, the clinical significance of this alteration remains unclear. -
