Genes affected
TPO (HGNC:12015): (thyroid peroxidase) This gene encodes a membrane-bound glycoprotein. The encoded protein acts as an enzyme and plays a central role in thyroid gland function. The protein functions in the iodination of tyrosine residues in thyroglobulin and phenoxy-ester formation between pairs of iodinated tyrosines to generate the thyroid hormones, thyroxine and triiodothyronine. Mutations in this gene are associated with several disorders of thyroid hormonogenesis, including congenital hypothyroidism, congenital goiter, and thyroid hormone organification defect IIA. Multiple transcript variants encoding distinct isoforms have been identified for this gene, but the full-length nature of some variants has not been determined. [provided by RefSeq, May 2011]

Pathogenic, criteria provided, single submitterclinical testingGeneDxFeb 05, 2019The R540X variant in the TPO gene has been reported previously in the homozygous state and in the heterozygous state with a second TPO variant in patients with congenital hypothyroidism; please note that this variant was referred to as c.1708C>T using alternate nomenclature by Bikker et al. (Bikker et al., 1995; Narumi et al., 2011; Cangul et al., 2015). This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The R540X variant is not observed at a significant frequency in large population cohorts (Lek et al., 2016). We interpret R540X as a pathogenic variant. -
Pathogenic, criteria provided, single submitterclinical testingInvitaeOct 22, 2023This sequence change creates a premature translational stop signal (p.Arg540*) in the TPO gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in TPO are known to be pathogenic (PMID: 11061528, 23236987, 25564141). This variant is present in population databases (rs121908082, gnomAD 0.003%). This premature translational stop signal has been observed in individual(s) with clinical features of thyroid dyshormonogenesis (PMID: 21900383, 34780050). ClinVar contains an entry for this variant (Variation ID: 4042). For these reasons, this variant has been classified as Pathogenic. -

Pathogenic, criteria provided, single submitterclinical testing3billionJan 03, 2022Stop-gained (nonsense): predicted to result in a loss or disruption of normal protein function through nonsense-mediated decay (NMD) or protein truncation. Multiple pathogenic variants are reported downstream of the variant (PVS1_VS). It is observed at an extremely low frequency in the gnomAD v2.1.1 dataset (total allele frequency: 0.000012, PM2_M). The variant has been reported to be associated with TPO related disorder (ClinVar ID: VCV000004042, PMID:7550241).Therefore, this variant is classified as pathogenic according to the recommendation of ACMG/AMP guideline. -
