NEB (HGNC:7720): (nebulin) This gene encodes nebulin, a giant protein component of the cytoskeletal matrix that coexists with the thick and thin filaments within the sarcomeres of skeletal muscle. In most vertebrates, nebulin accounts for 3 to 4% of the total myofibrillar protein. The encoded protein contains approximately 30-amino acid long modules that can be classified into 7 types and other repeated modules. Protein isoform sizes vary from 600 to 800 kD due to alternative splicing that is tissue-, species-,and developmental stage-specific. Of the 183 exons in the nebulin gene, at least 43 are alternatively spliced, although exons 143 and 144 are not found in the same transcript. Of the several thousand transcript variants predicted for nebulin, the RefSeq Project has decided to create three representative RefSeq records. Variant summary: NEB c.11627G>A (p.Trp3876X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. The variant allele was found at a frequency of 1.6e-05 in 248508 control chromosomes. c.11627G>A has been reported in the literature in at least one compound heterozygous individual affected with Nemaline Myopathy 2 (e.g., de Winter_2016). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 27074222). ClinVar contains an entry for this variant (Variation ID: 450119). Based on the evidence outlined above, the variant was classified as pathogenic. -

The W3876X variant is not observed at a significant frequency in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. Although the W3876X has not been previously reported to our knowledge, a different nucleotide substitution resulting in the same nonsense variant has been reported in a family with nemaline myopathy who harbored an additional NEB variant (Lehtokari et al., 2014). Additionally, other downstream nonsense variants in the NEB gene have been previously reported in the Human Gene Mutation Database in association with nemaline myopathy (Stenson et al., 2014). -
