Variant summary
Our verdict is Pathogenic. Variant got 14 ACMG points: 14P and 0B. PS1PM1PM2PM5PP2PP3PP5_Moderate
The NM_001165963.4(SCN1A):c.2862G>C(p.Glu954Asp) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★). Another nucleotide change resulting in same amino acid change has been previously reported as Likely pathogenicin ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. E954G) has been classified as Likely pathogenic.

Pathogenic, criteria provided, single submitter | clinical testing | Invitae | Jun 19, 2020 | This sequence change replaces glutamic acid with aspartic acid at codon 954 of the SCN1A protein (p.Glu954Asp). The glutamic acid residue is highly conserved and there is a small physicochemical difference between glutamic acid and aspartic acid. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SCN1A-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. A different variant (c.2862G>T) giving rise to the same protein effect observed here (p.Glu954Asp) has been determined to be pathogenic (Invitae). This suggests that this variant is also likely to be causative of disease. For these reasons, this variant has been classified as Pathogenic. - |
