This sequence change replaces glutamine, which is neutral and polar, with arginine, which is basic and polar, at codon 910 of the SCN1A protein (p.Gln910Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with autosomal dominant SCN1A-related conditions (PMID: 31273778; Invitae). ClinVar contains an entry for this variant (Variation ID: 422367). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SCN1A protein function with a positive predictive value of 95%. This variant disrupts the p.Gln910 amino acid residue in SCN1A. Other variant(s) that disrupt this residue have been observed in individuals with SCN1A-related conditions (PMID: 24168886, 28012175), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic. -

The Q901R variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The Q910R variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The Q910R variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. Additionally, this substitution alters a conserved position predicted to be within the transmembrane segment S5 of the second homologous domain. In silico analysis predicts this variant is probably damaging to the protein structure/function. Furthermore, missense variants at the same position (Q910K) and in a nearby residue (V907F) have been reported in the Human Gene Mutation Database in association with SCN1A-related disorders (Stenson et al., 2014).
