Variant summary: SCN9A c.4172T>C (p.Val1391Ala) results in a non-conservative amino acid change located in the Ion transport protein domain (IPR005821) of the encoded protein sequence. Three of four in-silico tools predict a damaging effect of the variant on protein function. Consensus agreement among computation tools predict no significant impact on normal splicing. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 6.5e-05 in 246156 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in SCN9A causing Channelopathy-Associated Congenital Insensitivity To Pain, Autosomal Recessive (6.5e-05 vs 0.0011), allowing no conclusion about variant significance. To our knowledge, no occurrence of c.4172T>C in individuals affected with SCN9A-related conditions and no experimental evidence demonstrating its impact on protein function have been reported. ClinVar contains an entry for this variant (Variation ID: 408582). Based on the evidence outlined above, the variant was classified as uncertain significance.

This sequence change replaces valine, which is neutral and non-polar, with alanine, which is neutral and non-polar, at codon 1391 of the SCN9A protein (p.Val1391Ala). This variant is present in population databases (rs200266532, gnomAD 0.02%). This variant has not been reported in the literature in individuals affected with SCN9A-related conditions. ClinVar contains an entry for this variant (Variation ID: 408582). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
