Variant summary
Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PM1PM2PM5PP3_StrongPP5_Very_Strong
The ENST00000373960.4(DES):c.347A>G(p.Asn116Ser) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. N116I) has been classified as Likely pathogenic.

Desmin-related myofibrillar myopathy Pathogenic:2
Likely pathogenic, criteria provided, single submitter | clinical testing | Laboratory for Molecular Medicine, Mass General Brigham Personalized Medicine | Mar 05, 2014 | The Asn116Ser variant in DES has been reported in 1 Caucasian individual with ea rly onset ARVC and was confirmed to be a de novo occurrence (Klauke 2010). Data from large population studies is insufficient to assess the frequency of this va riant. Functional studies have shown that the Asn116Ser variant impacts protein function (Klauke 2010, Brodehl 2012). However, these in vitro assays may not acc urately represent biological function. Computational prediction tools and conser vation analysis suggest that this variant may impact the protein, though this in formation is not predictive enough to determine pathogenicity. In summary, this variant is likely to be pathogenic, though additional studies are required to fu lly establish its clinical significance. - |
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Nov 29, 2022 | For these reasons, this variant has been classified as Pathogenic. Experimental studies have shown that this missense change affects DES function (PMID: 20829228, 22403400). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt DES protein function. ClinVar contains an entry for this variant (Variation ID: 66411). This missense change has been observed in individual(s) with DES-related conditions (PMID: 20829228, 26676851; Invitae). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 116 of the DES protein (p.Asn116Ser). - |
