COL4A3 (HGNC:2204): (collagen type IV alpha 3 chain) Type IV collagen, the major structural component of basement membranes, is a multimeric protein composed of 3 alpha subunits. These subunits are encoded by 6 different genes, alpha 1 through alpha 6, each of which can form a triple helix structure with 2 other subunits to form type IV collagen. This gene encodes alpha 3. In the Goodpasture syndrome, autoantibodies bind to the collagen molecules in the basement membranes of alveoli and glomeruli. The epitopes that elicit these autoantibodies are localized largely to the non-collagenous C-terminal domain of the protein. A specific kinase phosphorylates amino acids in this same C-terminal region and the expression of this kinase is upregulated during pathogenesis. This gene is also linked to an autosomal recessive form of Alport syndrome. The mutations contributing to this syndrome are also located within the exons that encode this C-terminal region. Like the other members of the type IV collagen gene family, this gene is organized in a head-to-head conformation with another type IV collagen gene so that each gene pair shares a common promoter. The p.Asn1508Ser change affects a highly conserved amino acid residue located in a domain of the COL4A3 protein that is known to be functional. In-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL) provide contradictory results for the p.Asn1508Ser substitution. This sequence change has previously described in an individual with tubulointerstitial kidney disease and in two individuals with a clinical diagnosis of Alport syndrome who also had variants in other Alport syndrome-related collagen genes (PMID: 36100708, 35675912, 27859054). Due to insufficient evidences and the lack of functional studies, the clinical significance of the p.Asn1508Ser change remains unknown at this time. -

The p.Asn1508Ser variant in COL4A 3 did not segregate in a sibling with hematuria and proteinuria, while the COL4 A4 variant was detected in that sibling (Fallerini 2017). This variant has also been identified in (52/126600) of European chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP rs200512461). Althoug h this variant has been seen in the general population, its frequency is not hig h enough to rule out a pathogenic role. Computational prediction tools and conse rvation analysis do not provide strong support for or against an impact to the p rotein.
