This sequence change replaces glycine, which is neutral and non-polar, with aspartic acid, which is acidic and polar, at codon 385 of the ALK protein (p.Gly385Asp). This variant also falls at the last nucleotide of exon 4, which is part of the consensus splice site for this exon. This variant is present in population databases (rs774127733, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with ALK-related conditions. ClinVar contains an entry for this variant (Variation ID: 665337). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.G385D variant (also known as c.1154G>A), located in coding exon 4 of the ALK gene, results from a G to A substitution at nucleotide position 1154. The glycine at codon 385 is replaced by aspartic acid, an amino acid with similar properties. This change occurs in the last base pair of coding exon 4 and may have some effect on normal mRNA splicing. However, loss of function of ALK has not been established as a mechanism of disease. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will not have any significant effect on splicing. This amino acid position is highly conserved in available vertebrate species. In addition, as a missense substitution this is predicted to be inconclusive by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
