This sequence change in SPAST is predicted to replace proline with serine at codon 509, p.(Pro509Ser). The proline residue is highly conserved (100 vertebrates, Multiz Alignments), and is located in a region involved in microtubule severing (PMID: 15269182). There is a moderate physicochemical difference between proline and serine. This variant is absent from the population database gnomAD v4.1. This variant has been detected in at least two probands with hereditary spastic paraplegia and cosegregates with the disease in two families (Invitae personal communication; Royal Melbourne Hospital). Computational evidence predicts a deleterious effect for the missense substitution (REVEL = 0.94). Based on the classification scheme RMH Modified ACMG/AMP Guidelines v1.7.1, this variant is classified as LIKELY PATHOGENIC. Following criteria are met: PP3_Strong, PM2_Supporting, PP1_Moderate, PS4_Supporting. -
This sequence change replaces proline, which is neutral and non-polar, with serine, which is neutral and polar, at codon 509 of the SPAST protein (p.Pro509Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of hereditary spastic paraplegia (internal data). It has also been observed to segregate with disease in related individuals. Invitae Evidence Modeling of clinical and family history, age, sex, and reported ancestry of multiple individuals with this SPAST variant has been performed. This variant is expected to be pathogenic with a positive predictive value of at least 99%. This is a validated machine learning model that incorporates the clinical features of 3,690 individuals referred to our laboratory for SPAST testing. ClinVar contains an entry for this variant (Variation ID: 1997879). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt SPAST protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic. -
