This variant is denoted MSH2 c.562G>C at the cDNA level, p.Glu188Gln (E188Q) at the protein level, and results in the change of a Glutamic Acid to a Glutamine (GAG>CAG). This variant was observed in at least one individual with a colon tumor that was microsatellite stable with MSH2 protein present on immunohistochemistry (Thompson 2013). MSH2 Glu188Gln was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Glutamic Acid and Glutamine differ in some properties, this is considered a semi-conservative amino acid substitution. MSH2 Glu188Gln occurs at a position that is conserved across species and is located in the connector domain (Lützen 2008). In silico analyses predict that this variant is probably damaging to protein structure and function. Based on currently available evidence, it is unclear whether MSH2 Glu188Gln is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. -
The MSH2 c.562G>C (p.Glu188Gln) variant has been reported in the published literature in an individual with colorectal cancer (PMID: 22949387 (2013)) and in an individual with a family history of breast and colon cancer (PMID: 35534704 (2022)). A published functional study reports that this variant is not damaging to protein function (PMID: 33357406 (2021)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

The p.E188Q variant (also known as c.562G>C), located in coding exon 3 of the MSH2 gene, results from a G to C substitution at nucleotide position 562. The glutamic acid at codon 188 is replaced by glutamine, an amino acid with highly similar properties. In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was reported to be functionally neutral (Jia X et al. Am J Hum Genet, 2021 Jan;108:163-175). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -

This sequence change replaces glutamic acid, which is acidic and polar, with glutamine, which is neutral and polar, at codon 188 of the MSH2 protein (p.Glu188Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with a personal or family history of colorectal cancer (PMID: 22949387, 35534704). ClinVar contains an entry for this variant (Variation ID: 422198). Invitae Evidence Modeling incorporating data from in vitro experimental studies (PMID: 33357406) indicates that this missense variant is not expected to disrupt MSH2 function with a negative predictive value of 80%. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
