Variant summary
Our verdict is Uncertain significance. The variant received 4 ACMG points: 5P and 1B. PM1PM2PP3BP6
The NM_000251.3(MSH2):āc.923G>Cā(p.Arg308Thr) variant causes a missense change. The variant allele was found at a frequency of 0.000000932 in 1,072,424 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R308K) has been classified as Uncertain significance.

Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -

not provided Uncertain:1
The MSH2 c.923G>C; p.Arg308Thr variant (rs1553352545), to our knowledge, is not reported in the medical literature but is reported in ClinVar (Variation ID: 489954). This variant is absent from general population databases (Exome Variant Server, Genome Aggregation Database), indicating it is not a common polymorphism. The arginine at codon 308 is weakly conserved, but computational analyses predict that this variant is deleterious (REVEL: 0.726). Due to limited information, the clinical significance of the p.Arg308Thr variant is uncertain at this time. -
Hereditary nonpolyposis colorectal neoplasms Uncertain:1
This sequence change replaces arginine, which is basic and polar, with threonine, which is neutral and polar, at codon 308 of the MSH2 protein (p.Arg308Thr). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with MSH2-related conditions. ClinVar contains an entry for this variant (Variation ID: 489954). Advanced modeling performed at Invitae incorporating data from internal and/or published experimental studies (PMID: 33357406) indicates that this missense variant is not expected to disrupt MSH2 function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
