Variant summary
Our verdict is Likely pathogenic. The variant received 7 ACMG points: 7P and 0B. PM1PM2PM5PP3
The NM_000251.3(MSH2):āc.1369A>Gā(p.Thr457Ala) variant causes a missense change. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. T457P) has been classified as Likely pathogenic.

Hereditary cancer-predisposing syndrome Uncertain:2
The p.T457A variant (also known as c.1369A>G), located in coding exon 8 of the MSH2 gene, results from an A to G substitution at nucleotide position 1369. The threonine at codon 457 is replaced by alanine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was reported to be functionally deleterious (Jia X et al. Am J Hum Genet, 2021 Jan;108:163-175). In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: MSH2 c.1369A>G (p.Thr457Ala) results in a non-conservative amino acid change located in the DNA mismatch repair protein MutS, core domain (IPR007696) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 250492 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1369A>G has been reported in the literature in at least an individual affected with pancreatic ductal adenocarcinoma (example: Cremin_2020). This report does not provide unequivocal conclusions about association of the variant with Lynch Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 32255556). ClinVar contains an entry for this variant (Variation ID: 491765). Based on the evidence outlined above, the variant was classified as uncertain significance. -
