The p.S516I variant (also known as c.1547G>T), located in coding exon 10 of the MSH2 gene, results from a G to T substitution at nucleotide position 1547. The serine at codon 516 is replaced by isoleucine, an amino acid with dissimilar properties. A study of the functional significance of MSH2 missense variants using CRISPR-Cas9 gene editing in human embryonic stem cells suggests this alteration has intermediate function utilizing DNA repair function, damage response signaling and protein stability functional assays (Rath A et al. Hum Mutat, 2019 11;40:2044-2056). In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was reported to be functionally neutral (Jia X et al. Am J Hum Genet, 2021 01;108:163-175). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -

To our knowledge, no occurrence of c.1547G>T in individuals affected with Hereditary Nonpolyposis Colorectal Cancer and no experimental evidence demonstrating its impact on protein function have been reported.
