Variant summary
Our verdict is Pathogenic. Variant got 10 ACMG points: 10P and 0B. PM1PM2PM5PP3_Strong
The NM_000179.3(MSH6):c.1238G>T(p.Trp413Leu) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. W413R) has been classified as Likely pathogenic.

Uncertain significance, criteria provided, single submitter | clinical testing | Invitae | Mar 26, 2021 | This variant has not been reported in the literature in individuals with MSH6-related conditions. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MSH6 protein function. This variant is not present in population databases (ExAC no frequency). This sequence change replaces tryptophan with leucine at codon 413 of the MSH6 protein (p.Trp413Leu). The tryptophan residue is highly conserved and there is a small physicochemical difference between tryptophan and leucine. - |
