The p.D575Y variant (also known as c.1723G>T), located in coding exon 4 of the MSH6 gene, results from a G to T substitution at nucleotide position 1723. The aspartic acid at codon 575 is replaced by tyrosine, an amino acid with highly dissimilar properties. This variant has been identified in at least one proband who met Amsterdam II criteria for Lynch syndrome and has been detected in probands whose tumors demonstrated high microsatellite instability and/or loss of MSH6 expression by immunohistochemistry (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -

ClinVar contains an entry for this variant (Variation ID: 216300). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MSH6 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This sequence change replaces aspartic acid, which is acidic and polar, with tyrosine, which is neutral and polar, at codon 575 of the MSH6 protein (p.Asp575Tyr). -
