This missense variant replaces glutamine with glutamic acid at codon 835 of the MSH6 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with breast cancer in the literature (PMID: 32019277, 32547938). This variant has been identified in 1/248314 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.Q835E variant (also known as c.2503C>G), located in coding exon 4 of the MSH6 gene, results from a C to G substitution at nucleotide position 2503. The glutamine at codon 835 is replaced by glutamic acid, an amino acid with highly similar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

The variant was also identified in dbSNP (ID: rs63751321) as "With Uncertain significance allele" and ClinVar (classified as uncertain significance by Ambry Genetics). The variant was identified in control databases in 1 of 244044 chromosomes at a frequency of 0.000004 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the East Asian population in 1 of 17164 chromosomes (freq: 0.00006), but not in the African, Other, Latino, European, Ashkenazi Jewish, Finnish, or South Asian populations. The p.Gln835 residue is conserved in mammals but not in more distantly related organisms and four out of five computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and 1 of 4 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time.
