This sequence change replaces lysine, which is basic and polar, with asparagine, which is neutral and polar, at codon 853 of the MSH6 protein (p.Lys853Asn). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with Lynch syndrome-associated cancer (Invitae). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 486911). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt MSH6 protein function with a negative predictive value of 95%. For these reasons, this variant has been classified as Pathogenic. -

The p.K853N variant (also known as c.2559G>C), located in coding exon 4 of the MSH6 gene, results from a G to C substitution at nucleotide position 2559. The lysine at codon 853 is replaced by asparagine, an amino acid with similar properties. This variant has been detected in multiple families meeting Amsterdam criteria and co-segregated with disease in at least one family and tumor results for several affected family members showed isolated loss of MSH6 expression on immunohistochemistry (IHC) (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
