This sequence change replaces asparagine with isoleucine at codon 984 of the MSH6 protein (p.Asn984Ile). The asparagine residue is weakly conserved and there is a large physicochemical difference between asparagine and isoleucine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with MSH6-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

PM2_Supporting, BP4 c.2951A>T located in exon 4 of the MSH6 gene, is predicted to result in the substitution of asparagine by isoleucine at codon 984, p.(Asn984Ile).It is not present in the population database gnomAD v2.1.1, non-cancer dataset (PM2_Supporting). Computational tools for this variant suggests no significant impact on splicing and does not affect the protein function (MAPP+PolyPhen-2 prior probability for pathogenicity: 0.0154)(BP4). To our knowledge, neither relevant clinical data nor functional studies have been reported for this variant. In addition, the variant has been reported in the ClinVar database (1x uncertain significance) but it has not been identified neither LOVD nor InSiGHT databases. Based on currently available information, the variant c.2951A>T is considered an uncertain significance variant according to ACMG guidelines. -
