Variant summary
Our verdict is Uncertain significance. Variant got 3 ACMG points: 4P and 1B. PM1PM2BP6
The ENST00000234420.11(MSH6):c.3964G>A(p.Glu1322Lys) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.000000685 in 1,460,340 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. E1322V) has been classified as Uncertain significance.

Hereditary cancer-predisposing syndrome Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Dec 08, 2022 | The p.E1322K variant (also known as c.3964G>A), located in coding exon 9 of the MSH6 gene, results from a G to A substitution at nucleotide position 3964. The glutamic acid at codon 1322 is replaced by lysine, an amino acid with similar properties. Using a Bayesian analysis that incorporates tumor mutation data, this variant was classified as likely benign (Shirts BH et al. Am J Hum Genet, 2018 Jul;103:19-29). In an in vitro complementation assay using MSH6 deficient nuclear extracts from cells, this variant was shown to be functional (Drost M et al. Genet Med, 2020 May;22:847-856). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
Lynch syndrome Benign:1
Likely benign, criteria provided, single submitter | clinical testing | University of Washington Department of Laboratory Medicine, University of Washington | May 01, 2018 | MSH6 NM_000179.2:c.3964G>A has a 1.8% probability of pathogenicity based on combining prior probability from public data with a likelihood ratio of 0.16 to 1, generated from evidence of seeing this as a somatic mutation in a tumor with loss of heterozygosity at the MSH6 locus. See Shirts et al 2018, PMID 29887214. - |
