DNMT3B (HGNC:2979): (DNA methyltransferase 3 beta) CpG methylation is an epigenetic modification that is important for embryonic development, imprinting, and X-chromosome inactivation. Studies in mice have demonstrated that DNA methylation is required for mammalian development. This gene encodes a DNA methyltransferase which is thought to function in de novo methylation, rather than maintenance methylation. The protein localizes primarily to the nucleus and its expression is developmentally regulated. Mutations in this gene cause the immunodeficiency-centromeric instability-facial anomalies (ICF) syndrome. Eight alternatively spliced transcript variants have been described. This sequence change replaces glutamic acid, which is acidic and polar, with glutamine, which is neutral and polar, at codon 761 of the DNMT3B protein (p.Glu761Gln). This variant is present in population databases (rs767814649, gnomAD 0.003%). This missense change has been observed in individual(s) with clinical features of immunodeficiency, centromeric instability and facial anomalies (ICF) syndrome (PMID: 31686314; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 572547). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt DNMT3B protein function with a positive predictive value of 80%. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

Variant summary: DNMT3B c.2281G>C (p.Glu761Gln) results in a conservative amino acid change located in the C-5 cytosine-specific DNA methylase (Dnmt) domain of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.2e-05 in 251468 control chromosomes. c.2281G>C has been reported in the literature in individuals affected with ICF Syndrome, Type 1 (examples: Kellner_2019, Internal data). These data indicate that the variant may be associated with disease. The following publication has been ascertained in the context of this evaluation (PMID: 31686314). ClinVar contains an entry for this variant (Variation ID: 572547). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -
