GDF5 (HGNC:4220): (growth differentiation factor 5) This gene encodes a secreted ligand of the TGF-beta (transforming growth factor-beta) superfamily of proteins. Ligands of this family bind various TGF-beta receptors leading to recruitment and activation of SMAD family transcription factors that regulate gene expression. The encoded preproprotein is proteolytically processed to generate each subunit of the disulfide-linked homodimer. This protein regulates the development of numerous tissue and cell types, including cartilage, joints, brown fat, teeth, and the growth of neuronal axons and dendrites. Mutations in this gene are associated with acromesomelic dysplasia, brachydactyly, chondrodysplasia, multiple synostoses syndrome, proximal symphalangism, and susceptibility to osteoarthritis. This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 380 of the GDF5 protein (p.Arg380Gln). This variant is present in population databases (rs397514668, gnomAD 0.0009%). This missense change has been observed in individual(s) with autosomal dominant brachydactyly type A2 (PMID: 18203755). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 40891). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt GDF5 protein function with a negative predictive value of 80%. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 250398 control chromosomes (gnomAD). c.1139G>A has been reported in the literature in multiple individuals affected with Brachydactyly type A2 (Ploger_2008). These data indicate that the variant is very likely to be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in 10%-<30% of normal activity (Ploger_2008). ClinVar contains an entry for this variant (Variation ID: 40891). Based on the evidence outlined above, the variant was classified as pathogenic. -
