The variant is observed at an extremely low frequency in the gnomAD v2.1.1 dataset (total allele frequency: 0.000%). Predicted Consequence/Location: Missense variant In silico tool predictions suggest damaging effect of the variant on gene or gene product [REVEL: 0.75 (>=0.6, sensitivity 0.68 and specificity 0.92)]. Same nucleotide change resulting in same amino acid change has been previously reported as pathogenic/likely pathogenic with strong evidence (ClinVar ID: VCV000040891 /PMID: 18203755). Therefore, this variant is classified as Likely pathogenic according to the recommendation of ACMG/AMP guideline. -

Published functional studies demonstrate decreased activity and reduced proteolytic cleavage (PMID: 18203755); Not observed at significant frequency in large population cohorts (gnomAD); This variant is associated with the following publications: (PMID: 33195419, 27563484, 18629880, 33333243, 29371961, 22828468, 28771427, 25092592, 31837199, 26275437, 18203755) -
This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 380 of the GDF5 protein (p.Arg380Gln). This variant is present in population databases (rs397514668, gnomAD 0.0009%). This missense change has been observed in individual(s) with autosomal dominant brachydactyly type A2 (PMID: 18203755). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 40891). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt GDF5 protein function with a negative predictive value of 80%. Experimental studies have shown that this missense change affects GDF5 function (PMID: 18203755). For these reasons, this variant has been classified as Pathogenic. -
Grebe syndrome Pathogenic:1
Variant summary: GDF5 c.1139G>A (p.Arg380Gln) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 250398 control chromosomes (gnomAD). c.1139G>A has been reported in the literature in multiple individuals affected with Brachydactyly type A2 (Ploger_2008). These data indicate that the variant is very likely to be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in 10%-<30% of normal activity (Ploger_2008). ClinVar contains an entry for this variant (Variation ID: 40891). Based on the evidence outlined above, the variant was classified as pathogenic. -
