Variant summary
Our verdict is Pathogenic. Variant got 12 ACMG points: 12P and 0B. PM2PP3_ModeratePP5_Very_Strong
The NM_020433.5(JPH2):c.482C>A(p.Thr161Lys) variant causes a missense change. The variant allele was found at a frequency of 0.00000143 in 1,399,574 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. T161T) has been classified as Benign.

Hypertrophic cardiomyopathy Pathogenic:1
This sequence change replaces threonine, which is neutral and polar, with lysine, which is basic and polar, at codon 161 of the JPH2 protein (p.Thr161Lys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with hypertrophic cardiomyopathy (PMID: 30235249, 33673806). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 155800). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). For these reasons, this variant has been classified as Pathogenic. -
