CBS (HGNC:1550): (cystathionine beta-synthase) The protein encoded by this gene acts as a homotetramer to catalyze the conversion of homocysteine to cystathionine, the first step in the transsulfuration pathway. The encoded protein is allosterically activated by adenosyl-methionine and uses pyridoxal phosphate as a cofactor. Defects in this gene can cause cystathionine beta-synthase deficiency (CBSD), which can lead to homocystinuria. This gene is a major contributor to cellular hydrogen sulfide production. This sequence change replaces alanine, which is neutral and non-polar, with glycine, which is neutral and non-polar, at codon 357 of the CBS protein (p.Ala357Gly). ClinVar contains an entry for this variant (Variation ID: 212880). A variant of uncertain significance has been identified in the CBS gene. The A357G variant has been reported to be benign based on an in vivo yeast assay designed to determine the effect of single amino acid substitutions on CBS enzyme activity. A357G was presumed benign as it did not impact yeast growth rates compared to wild-type (Dimster-Denk et al., 2013). The A357G variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. Nevertheless, this variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). This substitution occurs at a position that is conserved across species and in silico analysis predicts this variant is probably damaging to the protein structure/function. -
