Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. Of the 4 affected individuals, one of those was a homozygote, which increases the likelihood that the p.Arg217Ter variant is pathogenic (PMID: 12664304). This variant has also been reported in ClinVar (Variation ID#: 4130) and has been interpreted as pathogenic by Invitae, OMIM, and Institute of Human Genetics (University Hospital Muenster). This nonsense variant leads to a premature termination codon at position 217, which is predicted to lead to a truncated or absent protein. Loss of function of the HPS4 gene is strongly associated to autosomal recessive Hermansky-Pudlak syndrome 4. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive Hermansky-Pudlak syndrome 4.
