Variant summary
Our verdict is Likely benign. The variant received -5 ACMG points: 0P and 5B. BP4_StrongBP6
The NM_000551.4(VHL):c.115G>C(p.Gly39Arg) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000529 in 1,550,798 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 15/21 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G39V) has been classified as Uncertain significance.

The VHL c.115G>C; p.Gly39Arg variant (rs768650092) is reported in the literature in an individuals affected with a pheochromocytoma (de Cubas 2013), but without clear disease association. This variant is also reported in ClinVar (Variation ID: 411992), and is found in the general population with an overall allele frequency of 0.0092% (17/184822 alleles) in the Genome Aggregation Database (v2.1.1). Computational analyses are uncertain whether this variant is neutral or deleterious (REVEL: 0.193). Due to limited information, the clinical significance of this variant is uncertain at this time. In silico analysis supports that this missense variant does not alter protein structure/function; This variant is associated with the following publications: (PMID: 23660872)
Von Hippel-Lindau syndrome;C1837915:Chuvash polycythemia Benign:1
Hereditary cancer-predisposing syndrome Benign:1
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.
