This missense variant replaces serine with leucine at codon 183 of the VHL protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). A functional study has reported that this variant protein decreases stability compared to WT and may be partially active (PMID: 21454469). This variant has not been reported in individuals affected with VHL-related disorders in the literature. This variant has been identified in 1/251414 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

This sequence change replaces serine, which is neutral and polar, with leucine, which is neutral and non-polar, at codon 183 of the VHL protein (p.Ser183Leu). This variant is present in population databases (rs5030823, gnomAD 0.0009%). This missense change has been observed in individual(s) with erythrocytosis, pulmonary arterial hypertension (PAH) and/or paraganglioma (PMID: 21454469, 24466223). ClinVar contains an entry for this variant (Variation ID: 411985). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt VHL protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects VHL function (PMID: 21454469, 30338240). This variant disrupts the p. Ser183 amino acid residue in VHL. Other variant(s) that disrupt this residue have been observed in individuals with VHL-related conditions (PMID: 24466223; internal data), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.S183L variant (also known as c.548C>T), located in coding exon 3 of the VHL gene, results from a C to T substitution at nucleotide position 548. The serine at codon 183 is replaced by leucine, an amino acid with dissimilar properties. This alteration has been reported in an individual with a paraganglioma diagnosed at age 28 (Crona J et al. PLoS ONE 2014 Jan;9(1):e86756) and also in a compound heterozygous state (with VHL p.D126N) in an individual with hereditary erythrocytosis (Bond J et al. Blood 2011 Mar;117(13):3699-701). This variant was also detected in 1/370 Japanese pheochromocytoma/paraganglioma (PPGL) patients and classified as a variant of uncertain significance by the authors (Yonamine M et al. Cancers (Basel), 2021 Aug;13). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
