The encoded protein couples this information to intracellular signaling pathways that modify parathyroid hormone secretion or renal cation handling, and thus this protein plays an essential role in maintaining mineral ion homeostasis. While it is absent from the population and reported in an affected individual, the available evidence is currently insufficient to determine its role in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies. This variant has been reported in the literature in an individual affected with hypocalciuric hypercalcemia as a de novo (i.e. not present in the parents) variant (PMID: 22142470). This variant is not present in population databases (ExAC no frequency). This sequence change replaces glycine with arginine at codon 613 of the CASR protein (p.Gly613Arg). The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and arginine. -
