Genes affected
CCDC39 (HGNC:25244): (coiled-coil domain 39 molecular ruler complex subunit) The protein encoded by this gene is involved in the motility of cilia and flagella. The encoded protein is essential for the assembly of dynein regulatory and inner dynein arm complexes, which regulate ciliary beat. Defects in this gene are a cause of primary ciliary dyskinesia type 14 (CILD14). [provided by RefSeq, Jul 2011]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 6 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, product NOT destroyed by NMD, known LOF gene, truncates exone, which is 0.045647558 fraction of the gene. Cryptic splice site detected, with MaxEntScore 5.6, offset of 4, new splice context is: gctctctgtttgaccgttAGaat. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in inframe change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 3-180659582-T-G is Pathogenic according to our data. Variant chr3-180659582-T-G is described in ClinVar as [Pathogenic]. Clinvar id is 2180412.Status of the report is criteria_provided_single_submitter, 1 stars.

Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This sequence change affects an acceptor splice site in intron 5 of the CCDC39 gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in CCDC39 are known to be pathogenic (PMID: 21131972, 23255504). This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individuals with primary ciliary dyskinesia (PMID: 21131972, 23891469). For these reasons, this variant has been classified as Pathogenic. -
